We used the online immigration forms last week. Printed two sided. We were the only ones on our whole flight that had used them. The AA stewardess didn't even know about them. Also used the online customs form in link above that you can fill out within 3 days of the trip. Both forms worked great.

Were paper forms dispersed on the flight? The plane didn't have any for my last trip, and they were not easy to find on arrival.
 
They did handout immigration forms on the American Airlines flight from Dallas..
 
My family and I used the on line printable forms when we went to Cozumel in May. The website interface was a bit clunky, but it worked and the forms were accepted with no problems.
